Please submit your Veterinary Care Fund applications using our online form below. We’ll be in touch within two working days to notify you of the purchase order number assigned to this funding application.
You’ll then need to submit the pet’s medical history, client invoice and your invoice to Blue Cross for our contribution to [email protected] within six weeks of completing this form.
Please note that this application form is only for existing Veterinary Care Fund practice partners. If you are a veterinary practice and wish to register to access the fund, please click on the button below to be taken to our practice registration form.
Complete the form for each application you wish to make. You’ll receive an automated response summarising each application for your records.
